Scenarios:
1. A driver does not pass the whisper test in either ear. (Cannot hear a whisper from at least 5 feet.)
2. A driver has a completed Vision Evaluation Report (MCSA-5871) for monocular vision.

Discussion:
1. Drivers must meet the hearing standard in at least one ear to be certified. If they do not pass the whisper test in either ear, they must obtain an audiometric test before they can be certified. If one is not readily available at the time of the exam, the driver must be disqualified until one is acquired.
The hearing exemption is not applicable until a driver fails both the whisper and audiometric tests. When this is the case:
The examiner will mark “neither” in the hearing aid boxes on the exam form.
In the determination section, select the bullet “Meets standards, but periodic monitoring required”. Then in the space provided, write “hearing”.
On the next line, select that the driver is qualified for “other” and write in 2 years (if otherwise qualified).
Just below, check the box “Accompanied by a waiver/exemption” and write in “Federal Hearing”. Complete the certificate in the same manner.

2. The Vision Evaluation Report (MCSA-5871) is handled differently than the hearing and seizure exemptions. The box on the form and certificate that indicates certification must be accompanied by a waiver or exemption SHOULD NOT be marked, as it is not considered a waiver or exemption.
In the determination section, select the bullet “Meets standards, but periodic monitoring required”. Then in the space provided, write either “monocular vision” or “insulin-treated diabetes”.
On the next line, select that the driver is qualified for 1 year.

Conclusion:
1. The driver must obtain an audiometric test. If one is not readily available at the time of the exam, the driver must be disqualified until one is acquired.
2. The ME should first confirm that it has not been more than 45 days since the Vision Evaluation Report was completed. If not, the driver must have the eye specialist complete a new form. The ME then reviews the report, verifying that the driver meets the alternative vision standard.
A formal perimetry test must have been completed and a copy attached to the report
If the eye specialist indicates that the driver has either proliferative diabetic retinopathy or severe non-proliferative diabetic retinopathy, the driver should not be certified
The entire DOT physical exam is performed and if otherwise qualified, the driver can be certified for 1 year. The ME should keep the Vision Evaluation Report with the rest of their records.
