Expiration of FMCSA Exam Forms |
As most of you have noticed, several FMCSA examination forms are set to expire on 3/31/2025. FMCSA has previously published this statement: |
“Please note, the expiration date on [these forms] relates to the process for renewing the Information Collection Request that includes [these forms] with the Office of Management and Budget (OMB). The requirement to collect information as requested on [these forms] does not expire.”

To paraphrase, the forms themselves and the content contained in the forms do not expire. The expiration date in the top right corner is strictly for the OMB.

MEs should continue to use the forms that are currently posted on the FMCSA website until the renewal request has been approved. Upon approval, new versions of the Medical Examination Report Form MCSA-5875, Medical Examiner’s Certificate MCSA-5876, Insulin-Treated Diabetes Mellitus Assessment Form MCSA-5870, and the Vision Evaluation Report MCSA-5871 will be posted on the FMCSA website and in the TeamCME Member Center.

FMCSA notification from the last expiration date reset:
“Medical examiners should start using the forms with the 03/31/2025 date as soon as possible. However, MEs that have purchased these forms in bulk, or require time to reprogram electronic system forms, may continue to use the old forms until stocks have been depleted or forms have been reprogramed but should transition to use of the new forms as quickly as possible. Based on this guidance, SDLAs are encouraged to continue to accept these forms.”
